General Information

City of Great Falls is a self-funded health insurance (medical/Rx/dental/vision/EAP) program for the City of Great Falls eligible employees and dependents. The City’s current third party claims administrator and stop loss provider is Blue Cross and Blue Shield of Montana. The current Specific Stop Loss level is $200,000. They do not have aggregate stop loss enforce. The Health Plan offers one Managed Care/Point of Service benefit plan option for all employees.

Included in this Request for Proposal you will find eligibility information, current benefit descriptions and rate history, and the census.  Also included are experience information and high claims reports.

PLEASE RESPOND TO YOUR INITIAL PROPOSAL TO THIS REQUEST BY JULY 31, 2012. THE EFFECTIVE DATE OF THE CONTRACT WILL BE SEPTEMBER 1, 2012.
